Why Use Online Pre-registration?
Online pre-registration is designed for patients of Providence Imaging Center (PIC) who want to save time. The information requested is the same information that you would otherwise provide when you arrive at PIC the day of your procedure.

Does Online Pre-registration also schedule my appointment?
Please remember that online pre-registration does not schedule your imaging appointment. Please call us at (907) 212-3151 to schedule your mammogram, ultrasound, or other radiology exams. To schedule a CT, MRI, or PET-CT, please call (907) 212-3146. A convenient toll-free phone number is (888) 458-3151. Simply let us know that you've registered online, and we'll use that information to speed up the scheduling process.

How Do I Begin?
When you are ready, simply click in the check box below that you have read and agree to the terms below regarding your privacy and click the "Continue" button. You will then be directed to the secure "Providence Online Patient Pre-Registration" form. To begin to pre-register for an imaging study at PIC, select "Mammography" or "Providence Imaging Center" as the SERVICE YOU ARE REGISTERING FOR* in the drop down list on the top left of the online form. This ensures that your information comes directly to us. Then simply fill in the rest of the form!

What Happens When I'm Done?
When you're finished with the online form, simply click the "Submit" button. You will then be asked if you want to "Submit", "Edit", or "Print" the form. If everything looks okay, click "Submit" and the form will be on its way!

About Your Privacy
We are required by law to maintain the privacy of your medical information. We are also required to notify you of our legal duties and privacy practices regarding your medical information.

All information submitted to our Web site using our forms is encrypted for transmission using Secure Socket Layer (SSL) protocol and 128-bit encryption. If your Web browser does not currently support this security protocol, you will be prompted to update your security and presented with a link to a site where you may obtain the necessary plug-in at no charge. SSL is used for transmitting personal demographic information, personal health information and credit card Information.

Remember...
If you are scheduled for a procedure on a Monday, you cannot pre-register online or by phone after 5:00 pm on Friday. You will need to pre-register when you arrive, so please plan for the additional time.
