ProvPort users, alert! EpicCare Link has arrived, part of the new EMR that Providence Alaska Medical Center (PAMC) implemented on February 26, 2011. Now is the time to sign up your office's provider(s) and staff so that they will be able to access current patient records. Physicians and providers who are credentialed at PAMC should already have received a packet with the two new applications (as of 11/8/10). If you did not receive the applications, or you currently have ProvPort and want to use EpicCare Link, download the applications below. One form is an agreement for the entire office, and the other two forms are for each person who will need an account (provider, support staff, etc.).
Form for the entire office
Agreement for Access to Protected health Information in EpicCare Link- this is a pdf file, and is six pages long.

Non-employee individual application2 - this is a pdf file, and is one page long.
Forms for each individual office member Part II
