EpicCare Link - The Way to View Current Providence Records

EpicCare Link will be the permanent replacement for ProvPort on Feb. 26, 2011ProvPort users, alert!  EpicCare Link has arrived, part of the new EMR that Providence Alaska Medical Center (PAMC) implemented on February 26, 2011. Now is the time to sign up your office's provider(s) and staff so that they will be able to access current patient records. Physicians and providers who are credentialed at PAMC should already have received a packet with the two new applications (as of 11/8/10). If you did not receive the applications, or you currently have ProvPort and want to use EpicCare Link, download the applications below. One form is an agreement for the entire office, and the other two forms are for each person who will need an account (provider, support staff, etc.).

EpicCare Link Security Individual Access Request Form - this is a pdf file, and is 14 pages long. You only need to fill out and return the first two pages.
